Participant Waiver, Release of Liability, Assumption of Risk, and Indemnity Agreement

1. Acknowledgment and Assumption of Risk

I understand that participation in martial arts activities—including but not limited to striking, grappling, sparring, weapons training, fitness conditioning, and off-site events such as tournaments—carries inherent risks of physical injury, illness, property damage, or death. I voluntarily assume all such risks.

2. Waiver and Release of Liability

In consideration of being allowed to participate in any and all activities at Rising Warriors Martial Arts Academy Limited Liability Company, I release, discharge, and hold harmless the Academy, its owners, instructors, employees, volunteers, agents, successors, and affiliates from liability, claims, demands, causes of action, or expenses arising from participation.

3. Indemnification

I agree to indemnify and hold harmless the released parties from any claim brought by me, my child, or anyone on our behalf.

4. Medical Treatment Consent

I authorize the Academy to administer first aid and/or secure emergency medical treatment if necessary. I accept full financial responsibility for medical care and transportation.

5. Photo & Video Release

I grant permission for photographs, videos, and recordings of myself and/or my child to be used in promotional materials, social media, and advertisements without compensation.

6. Minor Clause

As parent or legal guardian of the named minor, I understand and accept all terms of this agreement on their behalf.

7. Severability

If any provision is held invalid, the remainder shall continue in full legal force and effect. This agreement is governed by the laws of the State of Florida.

8. Entire Agreement

This waiver constitutes the full agreement between the parties.
